Cardiology

Cardiology [Greek: kardia, heart + logos, a discourse] Erasistratus who lived around 300 BC, believed that the valves of the heart directed the blood towards the veins. The ancient Chinese lacked precise anatomical knowledge of the heart and they believed that the trachea opened into the heart. The Roman physician Galen(129–200) proposed that blood from the left ventricle reached the right side of the heart via pores in the interventricular septum. His teaching was accepted for over 1000 years until William Harvey(1578–1657) announced his discovery of the circulatory system in 1628. David Dundas, a British physician in the late 17th century who did pioneer work on rheumatic diseases, was one of the first to concentrate on the heart and he published An Account of Peculiar Diseases of the Heart in 1808. The ëpeculiar diseaseí described by him was later recognized as rheumatic heart disease. A description of coronary circulation was given by Raymond Vieussens (1641–1715) of France in his Traite du Coeur in 1715. The calcification of the coronary arteries was described by Giovanni Battista Morgagni (1682–1771) in 1761. David Pitcairn (1749ñ1809), a physician at St Bartholomewís Hospital, was one of the first to concentrate on the pathology of the heart. He noted the lesions in the heart valves following rheumatic fever and introduced the term 'rheumaticí in the description of heart disease in 1788. Jean Nicholas Corvisart (1755ñ1821) wrote an important treatise on the diseases of the heart in 1814. English cardiologist, Sir James Mackenzie (1853ñ1925), developed his own ink polygraph by which he simultaneously recorded arterial and venous pulsations during his study on arrhythmias. He was the first to note the loss of effective atrial contraction or ëatrial paralysisí which was later named ëatrial fibrillationí by Sir Thomas Lewis (1881–1945). Mackenzie also described the functional pathology of cardiac tissue in 1907 and established the place of digitalis in the treatment of auricular fibrillation. The first electrocardiogram of a man was recorded by August Desire Waller (1856–1922), a physiologist at St Maryís Hospital in London, in 1887.  Dutch physiologist, Willem Einthoven (1860–1927), simplified the method with his invention in 1903 of the string galvanometer, which developed into the electrocardiograph. His apparatus was improved by Lewis who added two strings instead of a conventional single string, making it possible to record different leads. The first recording of a fetal electrocardiogram was reported by M. Cremer of Germany in 1906. Alexis Carrel (1884ñ1947), a French-born American biologist, reported the first heart transplantation in a dog in 1905, and the first human heart transplantation was achieved by Christian Neethling Barnard (b-1922) at Groote Schuur Hospital in Cape Town in 1967. Cardiac catheterization was performed by Werner Forssmann (1868–1941) of Germany on himself in 1929. A practical electrical defibrillators to treat the irregularity of the heart was constructed by an American professor of electrical engineering, William Bennett Kouwenhoven (1886ñ1975) of Johns Hopkins University, in 1936. An attempt at bypassing diseased coronary arteries by implanting pectoral muscles onto the pericardium was made by Claude Schaeffer Beck (1894–1971) of America in 1935. Other advances in cardiac surgery made in the United States were made by: William Wayne Babcock (1872–1963), Beck (1894–1971), Alfred Blalock (1899–1964), Robert Edward Gross (1905–1988), Michael Ellis DeBakey (b 1980 and Arthur Denton Cooley (b 1920). Rene G. Favaloro, a rural physician from La Plata, Argentina performed the first successful coronary artery bypass graft using the saphenous vein, on a woman who had an obstructed right coronary artery, at the Cleveland Clinic in 1967.
