Gastrointestinal Surgery

Gastrectomy [Greek: gaster, stomach + ektome, excision] Resection of the stomach for carcinoma was performed by Emile Jules Pean (1830–1898) of Paris in 1879, but was not successful. In 1881 Christian Albert Theodor Billroth (1829–1894) performed the first successful gastric resection. A complete resection of the stomach was performed by Phineas Sanborn Conner of Cincinnati in 1884, but this was unsuccessful. Carl Bernhard Schlatter (1864–1934), a German surgeon, performed the first successful total gastrectomy for carcinoma of the stomach in 1897. After this, a total of 70 cases, including one procedure of his own, were reported by W. Walters of America in 1929. A procedure of partial gastrectomy for carcinoma of the pyloric end of the stomach was devised by William James Mayo (1861–1939) in 1900. See Billroth operation. Postmortem diagnosis of gastric carcinoma in the 18th and 19th centuries, were observed by Giovanni Battista Morgagni (1682–1771) in 1761, Matthew Baillie (1761–1823) in 1793, Carl von Rokitansky (1804–1878) in 1842, Gustav Hauser in 1883 and William Henry Welch (1850–1934) in 1885. One of the best descriptions of autopsy findings was given by Hauser, and the best statistical study was done by Welch. The first successful gastroenterostomy for gastric carcinoma was performed by Anton Wolfer (1850–1917) in 1881. William Osler (1849–1919) reported 24 advanced cases of stomach cancer in 1894, out of which only one was submitted to surgery (with a fatal outcome). Until the beginning of the 20th century it was rare to see or diagnose gastric cancer before it became terminal. Osler reported his clinical experience in 150 patients in 1900. X-ray studies were employed in diagnosis around 1914. In 1909 William Carpenter McCarty (b 1880) of the Mayo Clinic, following his study on the cytology of gastric ulcers in 1907 proposed that chronic gastric ulcers were potentially precancerous. Gastric Fistula Caused by a gunshot wound in a patient, this was used by American surgeon, William Beaumont (1785–1853), to study secretions of the stomach in 1822. The first operative treatment was performed by Albrecht Theodor Middeldorpf (1824–1868) in 1859. See fistula.
