Gynecology, Gynaecology

Gynecology [Greek: gyne or gyne, woman + oeikos, house + logos, discourse] Soranus of Ephesus (AD98–138), who practiced in Alexandria and later in Rome during the reign of Hadrian and Trajan, is regarded as the founder of obstetrics and gynecology. His De Morbis Mulierium  remained a standard work for 15 centuries. It became a specialty only in the 18th century. Previously, many surgeons dealt with the gynecological problems. Ambroise Paré (1510–1590), the celebrated French surgeon of the Renaissance, attempted to treat vesico-vaginal fistula, and wrote treatises related to gynecology. Caspar Bauhini(1560–1624), professor of anatomy and medicine at Basel, also wrote important treatises. Various structures of female genital tract, such as the oviducts or Fallopian tubes (discovered by Gabriele Falloppio in 1561), Graafian follicles (discovered by Regnier de Graaf in 1672), and the vulvovaginal glands (discovered by Caspar Bartholin in 1677) were described during this period. The first ovariotomy was performed at Danville, Kentucky by Ephraim McDowell(1771–1830) of Virginia in 1809. The procedure was introduced into England by Charles Clay (1801–1893) of Manchester and Sir Thomas Spencer Wells (1818–1897).James Marion Sims (1813–1883) of South Carolina was one of the first to establish gynecology as a specialty, around 1840. He pioneered surgery in treatment of vesico-vaginal fistula, and designed the first vaginal speculum to visualize it. His device developed into the Sims  duck-billed speculum. The Women’s Hospital in New York was founded by Sims in 1853, and the uterine sound was invented by him in 1843. Myomectomy as treatment for fibroids of the uterus was performed by Washington Lemuel Atlee(1808–1878) in 1844. Uterine myoma was surgically removed by Italian surgeon, Augosto Granville Bozzi (1782–1872), in 1827, but without success. Successful abdominal hysterectomy was performed by American surgeon, Walter Burnham (1808–1883) in 1853, and another American surgeon, Gilman Kimball (1804–1892), performed the second in 1855. Wells performed his first hysterectomy for myoma in 1861. An early American classic, A Practical Treatise on the Diseases of Women, was published by Theodore Thomas Gaillard (1832–1903) of Edisto island South Carolina in 1868, who was the first to perform vaginal ovariotomy in 1870. A radical form of hysterectomy for carcinoma was performed by an American gynecologist, John Goodrich Clark (1867–1927) of Indiana in 1895. Abdominal hysterectomy was introduced as treatment of cancer by German surgeon, Wilhelm Alexander Freund (1833–1918) in 1913. The Wertheim radical pan-hysterectomy for cervical cancer was devised by an Austrian gynecologist, Ernest Wertheim (1864–1920) in 1898. Enucleation of uterine fibroids by a vaginal route was devised by Vincenz Czerny (1842–1915), professor of surgery at Freiburg, in 1881. Recognition of the role of hormonal factors in the female reproductive cycle by Eugen Steinach (1861–1944) in 1911, Edgar Allen (1892–1943) and Edward Adelbert Doisy (b 1893) in 1923, Bernhard Zondek (b 1891) in 1927, and Adolf Friedrich Johann Butenandt (b 1903) in 1929, led to better understanding and treatment. Study of exfoliative cytology related to normal cyclical changes of the vaginal epithelium by Nicholas George Papanicolau (1883–1962) in 1917 led to mass screening for cervical cancer.
