Therapeutics

THERAPEUTICS [Greek: therapeutikos, inclined to serve] Effective herbal remedies were known in China at the time of Emperor Shen Nung 5000 years ago. Priests in the temple of Aesculapius practiced medicine through mysticism and magic, which later gave way to physical medicine with mineral baths, massage, blood letting and use of substances such as iron, milk and honey. Bizarre and unusual remedies such as crocodile dung, burnt human bones, blood from a stabbed gladiator, urine of an adolescent boy, owl blood, ass liver and similar substances were used by Greeks and Romans up to the 17th century. Galen(AD 129–200) listed over 300 herbal medicines and Pedanius Dioscorides, a Greek physician from Anarzaba during the reign of Nero, studied medicinal herbs and described over 600 plants in five books on materia medica. Paracelsus (1493–1541), physician, alchemist, philosopher and astrologer from Switzerland introduced chemical therapeutics, including mercurials. A landmark in the therapeutics was the introduction of cinchona as treatment for malaria by a Spanish physician, Juan del Vego in 1639. Modern pharmacology began in the early 19th century with the discovery and isolation of plant alkaloids such as morphine by Frederick Wilhelm Sertuerner (1783–1841), emetine by Pierre Joseph Pelletier (1788–1842) and quinine from cinchona by Joseph Bienaimé Caventou (1795–1877) in 1824. Chemotherapeutics was established by Paul Ehrlich (1854–1915)with his concept of side chain theory. In 1910 he developed salvarsan or arsphenamine for treatment of relapsing fever, syphilis and trypanosomiasis. The battle against bacteria was won with the discovery of antibiotics by Nobel Prize winner,Gerhard Johannes Paul Domagk (1895–1964) in 1935 and penicillin by Sir Alexander Fleming (1881–1955) in 1929 and demonstration of its clinical effectiveness by Sir Ernest Boris Chain (1906–1979) in 1940. They shared the Nobel Prize with Howard Walter Florey(1898–1968)  in 1945 for their work on penicillin.  Hydrotherapy is still used today. Psychotherapy began with Johann Christian Reil (1759–1813) and was established by Sigmund Freud (1856–1939),  Carl Gustav Jung (1875–1961) and others. Occupational therapy was also known in ancient times and Seneca who lived in 30 BC recommended employment for treatment of mental unrest. Application of physical medicine to diseases of joints and bones was pioneered by French surgeon, Amédée Bonnet(1802–1858) around 1845. Antimetabolites such as mercaptopurine, methotrexate, and cyclophosphamide are used in cancer and immunosuppressive therapy. Alternative therapies such as aromatherapy, homeopathy and acupuncture are also used. See chemotherapy, pharmacology, alternative therapy.
