Tuberculosis

TUBERCULOSIS Disease affecting the lungs, known as phthisis or consumption before the discovery of the tubercle bacillus, Mycobacterium tuberculosis, by Robert Koch (1843–1910) in 1882. Percivall Pott (1714–1788), a surgeon at St Bartholomew’s Hospital in London, described spinal caries in 1779 but probably did not realize its tuberculous nature. A description of the pathological appearance of the tubercle in the lung was given by Matthew Baillie(1761–1823) in Morbid Anatomy of Some of the Most Important Parts of the Human Body published in 1793. Gaspard Laurent Bayle (1774–1816), a physician from Provence who practiced at Paris, gave pathological findings in Recherches sur la Phthisie Pulmonaire published in 1810. Before the identification of a bacterium various remedies, such as sea air and warm climate, were recommended as treatment. Induction of artificial pneumothorax as treatment was introduced by James Carson (1772–1843) of Liverpool in 1821 and used for the next 100 years. Other forms of collapse therapy, such as phrenic crush, apicolysis and thoracoplasty, were also used. Secret remedies were common before the advent of chemotherapy and included Tuberculozyme, Brompton Consumption mixture and Steven’s Consumption Cure. The infectious nature was demonstrated by French physician, Jean Antoine Villemin (1827–1892) who inoculated an animal with material taken from the lung of a patient with tuberculosis in 1865. A tuberculosis dispensary was established at Edinburgh bySir Robert William Phillip(1857–1939) of Edinburgh Royal Infirmary in 1888. He pioneered study and control and established a tuberculosis dispensary in London at Paddington in 1909. Creosote, obtained from wood, was a popular remedy for the next 50 years and was revived as a specific remedy by Charles Jacques Bouchardt (1837–1915) in 1877. The drug Iproniazid was used in the treatment in 1910 and streptomycin was introduced as first line drug by Hortin Corwin Hinshaw (b 1902) in 1946. Isoniazid was used by Edward Robitzek in 1952 and given in combination with pyrazinamide by Walsh McDermottand colleagues in America in 1954. Cycloserine was introduced by P. B. Storey in 1958, and ethionamide was added by Noel Rist in 1959. Ethambutol was used by J. P. Thomasand co–workers in 1961.
